Liability Waiver for Club Diving
In order to participate in any diving event with Big Apple Divers, we must have a digital copy of a liability waiver for the current year on file with the Membership Secretary. We do not presently have the capability to work with digitally signed legal documents, so this must be printed out completed by hand.
The waiver MUST be hand initialed on the first page, and the second page must be completed, INCLUDING the witness section. You can give us an original copy, or email a PDF of the completed document (not a JPEG) to membership@seagypsies.nyc.
